Please submit the following to our office:

• Copy of Application for Benefits VA Form 22-5490
• Copy of your Certificate of Eligibility (COE)
• Copy of your birth certificate/marriage license or other documentation that establishes your relationship to the veteran

Transfer Students

If you have used benefits at another institution, please submit the following forms to our office:

• Copy of Request for Change of Program or Place of Training VA Form 22-5495
• Copy of your Certificate of Eligibility (COE)
• Copy of your birth certificate/marriage license or other documentation that establishes your relationship to the veteran
